Application process

Required documents for admission process in 2013/2014:


Candidates for the Transatlanitc Studies Programme must open and complete the application form in the Jagiellonian University Candidate Registration System.

I . When applying for Transatlantic Studies all candidates in order to confirm their registration are required to submit the following documents:

 

1) Scanned document of application form printed from the individual OAS account and signed by the candidate. Online application

2) Scanned document of the higher education diploma (at least undergraduate) or other document of higher education graduation obtained abroad, certifying the holders right to apply for graduate programmes in the country of issue, considered to be equivalent to respective Polish higher education diploma of at least undergraduate programme, and when the candidate does not have one yet upon registration, a certificate issued by the granting institution confirming completion of higher education final examination (diploma examination) and containing information of holders right to apply for graduate programmes in the country of issue, together with final examination grade, diploma dissertation grade and higher education diploma grade and grade point average established according to granting institution regulations,

Scanned document of diploma supplement or official transcript and if the candidate does not have it yet, the photocopy of a students record book (indeks) or other document including a full list of courses completed and grades received,


4)
Scanned document of both sides of identification card and in case of non-Polish citizens scanned document of a passport page containing the photograph,

5) Two confidential reference letters, each must be submitted in an envelope signed across the seal by the referee ( at least one must be academic and the other should be either academic or related to the application, i.e. not a general character reference). Should be sent directly to TAS Office

6) Curriculum Vitae (résumé) in the Europass format (available here )

7) Application fee of 18,00 EUR/ 25,00 USD/ 75 PLN ( non-refundable)

8) A completed copy of the Europass Language Passport. Form and instructions can be found here

***Sworn translation into English or Polish of all the above documents if they were issued in language other than English or Polish



Candidates who hold higher education diploma obtained outside Poland are required to submit additionally the following documents:

* the documents of higher education graduation obtained abroad are required to be authenticated with apostille (if the document is issued in one of the signatory countries of Convention of 5 October 1961 Abolishing the Requirement of Legalisation for Foreign Public Documents) or in other cases legalised.

 




II. Once admitted to the Transatlantic Studies Program, the candidate will be required to provide the following aditional documents:


Documents required upon enrolment:


1) Original application form printed from the individual OAS account and signed by the candidate,


2) Photocopy of Polish maturity certificate or International Baccalaureate diploma (IB) or European Baccalaureate certificate or secondary-school leaving examination certificate or other document obtained abroad considered to be equivalent to respective Polish maturity certificate (together with its original available upon request),

 


***Maturity certificate or other document obtained abroad considered to be equivalent to respective Polish maturity certificate, other than the IB diploma or EB certificate, are required to be authenticated with apostille or legalised,


4) Photocopy of the higher education diploma (at least undergraduate) or other document of higher education graduation obtained abroad, certifying the holders right to apply for graduate programmes in the country of issue, considered to be equivalent to respective Polish higher education diploma of at least undergraduate programme (together with its original upon request), or when the candidate does not have one yet upon enrolment, a certificate issued by the granting institution confirming completion of higher education final examination (diploma examination) and containing information of holders right to apply for graduate programmes in the country of issue, together with final examination grade, diploma dissertation grade and higher education diploma grade together with written declaration to submit the photocopy of the document (with its original available upon request) by a given deadline - under the pain of withdrawal from the university,

 


5) Photocopy of diploma supplement (with its original available upon request) or official transcript, or when the candidate does not have one yet upon enrolment - the candidates written declaration to submit the photocopy of the document (together with its original available upon request) by a given deadline - under the pain of withdrawal from the university,


6) Three photographs (35mm x 45 mm) taken against a light background without head covering (according to the guidelines for issuing identity cards),


7) Photocopy of one of English language proficiency certificates:
TOEFL (Paper-based - 550, Internet-based - 79, Computer-based: 213),
Cambridge IELTS written version - 6.5,
CAE - B grade,
CPE - C grade
 (does not apply to native English speakers and candidates who completed their higher education in English),


8) Photocopy of both sides of identification card and in case of non-Polish citizens scanned document of a passport page containing the photograph (together with its original available upon request),


 **** Sworn translation into English or Polish of all documents submitted if they were issued in language other than English or Polish.

 

 

Candidates who do not hold Polish citizenship are required to submit additionally the following documents upon enrolment:



9) Photocopy of a medical certificate that confirms the lack of health conditions disqualifying the candidate from undertaking study programmes (together with the original available upon request).


10) Photocopy of document that certifies the candidates legal stay in the territory of Poland, i.e. visa or a temporary/permanent residence permit or other document that allows for stay in the Republic of Poland (together with the original available upon request),


11) Photocopy of document that certifies the candidates possesion of health insurance or casualty insurance for the period of studying in Poland, or photocopy of the European Health Insurance Card (together with the original available upon request), or a declaration of taking out the insurance in the National Health Fund (Narodowy Fundusz Zdrowia) immediately upon start of first semester,

   


Candidates who hold higher education diploma obtained outside Poland are required to submit additionally the following documents:


* nostrification certificate issued by the board of the faculty offering the given programme of a respective Polish higher education institution, or the applicants written declaration to nostrify the higher education diploma by the end of the first semester under the pain of withdrawal from the university - it does not concern documents that were issued by the authorized institution of the European Union country or the European Free Trade Association (EFTA) country – a party to the Agreement on the European Economic Area or the country – a party of The Convention on the Recognition of Qualifications concerning Higher Education in the European Region signed in Lisbon on 11th April 1997 or countries with which Poland have signed an agreement on mutual recognition of education,

 


* the documents of higher education graduation obtained abroad are required to be authenticated with apostille (if the document is issued in one of the signatory countries of Convention of 5 October 1961 Abolishing the Requirement of Legalisation for Foreign Public Documents) or in other cases legalised.
